We understand that returning home from hospital can be a very stressful time for both patients and family members.
Preparation and co-ordination must occur to ensure the patient travels safely home and that everything they need is waiting there for them.
A patient may need medication dispensed, appointments booked, and any hospital issued home rehabilitation instructions to be managed.

Chiltern Home Care can provide a registered nurse or experienced caregiver (depending on requirements) to meet with a patient and their family at the hospital to discuss and establish a plan for the Patients discharge.
We will take time to understand the individual needs and help remove the stress of discharge and help to ensure the patient a successful recovery.

Hospital discharge planning is intended to decrease the length of stay in hospital as well as to ensure continuity of care so as to decrease the chance of re-admission.
In the majority of cases, when a patient is judged by the hospital to be fit for discharge, transfer home can be arranged very quickly.



Respite Care



Respite care is a chance for you and the person you care for to take a break.
As a carer you may need a break for a few hours or a few days. Moreover the person for whom you are caring might need a change of scenery or want more companionship than you can provide.

Making arrangements to ensure excellent care in place over a holiday or respite period doesn’t need to be difficult.

Chiltern Home Care can provide respite care ranging from a few hours per week to 24 hour live in care. We can provide transport to an appointment or social outing, shopping, bill payment or by providing comforting and caring companionship to your loved one.
